Evidence-based interventions to prevent and manage type 2 diabetes

As a provider of the Healthier You: NHS Diabetes Prevention Programme, we're helping thousands of patients across the country make changes to their lifestyle and prevent the onset of type 2 diabetes.

We're also supporting individuals living with type 2 diabetes, with 40% of patients achieving HbA1c below the diagnostic threshold for diabetes (<48 mmol/mol). Additionally, we're also part of Wave 2 of the NHS Test Beds project, where we're delivering a new approach to diabetes management across Hampshire and Farnham.

Our results speak for themselves


With a 79% completion rate, people using the programme achieved an average 8.3% weight loss at 6 months (about 7 kg), an average HbA1c reduction of 13.6 mmol/mol at 3 months and 40% lowered their HbA1c below the diagnostic threshold for diabetes. Our outcomes are published in the peer-reviewed Diabetic Medicine, Advanced Technologies & Treatments for Diabetes and Future of Healthcare Journal.

We have on-going research in collaboration with Imperial College Health Partners and UCL as part of the NHS Diabetes Prevention Programme.

Our work in Portsmouth delivered significant weight loss and HbA1c outcomes for people living with type 2 diabetes. Patients were referred by practice and specialist nurses resulting in 79% of individuals enrolling following referral.

Comparing our commercial and NHS participants, 41% of those referred via the NHS were male. Even in this traditionally hard to reach group, significant weight loss outcomes were achieved.

In our initial study we were able to achieve and sustain clinically significant weight loss, exceeding the 5-7% weight loss outcomes that are associated with >50% reduction in risk of type 2 diabetes.

Managing type 2 Diabetes in North West London


In 2017, OurPath became the first ever online behavioural change programme to be commissioned by the NHS.

We delivered our programme to hundreds of patients living with type 2 diabetes in 6 CCGs across north-west London, helping them change their behaviours to better manage their condition. Find out more about the results of our first NHS pilot – or hear it straight from our patients themselves.
